Once enrolled in the plan, employees will receive a member card at the mailing address on file with the District. When visiting a provider, employees can also provide their Social Security number for verification.

Only employees are eligible for a second pair of glasses every 24 months. A second pair of glasses (frames up to $115 with the same covered lens options as the first pair EXCEPT progressives) OR contacts (up to $105 allowance).
